Shock is a life-threatening condition of circulatory failure due to reduced oxygen delivery, increased oxygen consumption, inadequate oxygen utilization, or a combination of these processes, which produces cellular and tissue hypoxia. The effects of shock are initially reversible, but rapidly become irreversible, resulting in multiorgan failure and even death. The four types of shock are recognized as distributive, cardiogenic, hypovolemic, and obstructive, however, patients with circulatory failure have a combination of more than one form of shock. Distributive shock is caused by severe peripheral vasodilatation and includes septic shock, SIRS, neurogenic shock, anaphylactic shock, endocrine shock, and drug/toxin induced shock. Cardiogenic shock is due to intracardiac causes of pump failure with resulting reduced cardiac output. This type of shock includes cardiomyopathic, arrhythmic, and mechanical shock. Hypovolemic shock is characterized as reduced intravascular volume or reduced preload, which reduces cardiac output. The two categories are hemorrhagic and nonhemorrhagic shock. Obstructive shock is mostly due to extracardiac causes of cardiac pump failure and often associated with poor right ventricular output; this includes pulmonary vascular and mechanical shock.

Adherence to medication can be linked to taste in the pediatric population. Most children will not take their medicine due to unpleasant taste. Here are some counseling tips to help improve medication adherence.

DKA is an acute complication of diabetes. With the help of pharmacists, we can prevent this from happening and enlighten our patients with some tips. Pharmacists need to counsel their patients on the importance of compliance with insulin, as well as a healthy diet. Non-adherence to insulin and inadequate food intake can lead to DKA. Lastly, remind them of the warning signs of DKA such as the 3 P’s: polyuria, polydipsia and polyphagia.

Happy World Health Day!! • Before pharmacy school, I worked as a Pharmacy Technician for 7 years w/ Fred Meyer. After getting my Intern license in Oregon, they transitioned me into the Pharmacy Intern position - which is my current position w/ the company. • Never, ever take your technicians foregranted. To all my classmates and pharmacy school peers, if you don’t already prioritize this fact, I highly recommend it. • Many of us have had the opportunity to work from both sides of the white coat in pharmacy, and I believe it is our duty to educate those of us who have not been that fortunate. • Your pharmacy technicians are *YOUR EQUALS*, or - tbh - are probably more knowledgeable than you (a lowly intern). VALUE your technicians, when you’re making that PharmD salary - treat them out to coffee every once in a while, and before that - treat them w/ every bit of respect as you would for your preceptor. • Your pharmacy technicians have the ability to MAKE or BREAK your day. They are as essential as the pharmacist in the pharmacy. Appreciate your Tech Tuesday is a tradition I want to start when I’m working. • My pharmacy managers have always been awesome, so I’ve been very lucky but I’ve come across some pharmacists that don’t treat technicians as a valued member of the team - don’t be one of those! Your team’s hard work will reflect the quality of work environment you provide!!! • ⛑

Research shows that antibiotics are prescribed to patients when they often shouldn’t be. They are often needlessly thrown at a wide array of medical complications, including viral illnesses such as the flu and common cold. This abundance of use is not only a waste of resources, but it also contributes to antibiotic resistance. With each inappropriately prescribed antibiotic, bacteria become more and more resistant, complicating future therapy. A CDC report on antibiotics estimates that 2.8 million Americans survive antibiotic-resistant infections every year. With the improper use of antibiotics and the complications becoming increasingly difficult to treat, patients may experience extended hospital stays while the doctors are forced to weigh alternative, possibly more hazardous therapy options.

Antimalarial agent that works by interfering with the digestive function in malarial parasites. 2. As far as #coronavirus is concerned, it’s thought to change the pH at the cell membrane and so inhibit viral fusion 3. A short few weeks from today I will be taking the last of my final exams and preparing for a year of rotations. Crazy right? . ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ A lot of things seem crazy at this moment in time and for most of us (especially those of us working in the medical field) there is an unsettling feeling and fear of the unknown. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ . ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ But we can only control so many variables and the rest is out of reach. Things always get worse before they get better. & they will get better if we do our part. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ . ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ Stay home. Wash your hands (multiple times daily). If you HAVE to go out, cover your face with a bandana or homemade cloth mask and sanitize/wash your hands before and after to reduce the chance of spreading the virus. Use the drive thru at your pharmacy. Check in on your neighbors and loved ones (via text or facetime of course). & lastly, find a reason to smile during this time. Looking back on old pictures and this was just about a year ago, when @cliffonedge and I spent World TB Day educating people in front of our lower campus library! • Tuberculosis (TB) is especially important in the state of Hawaii due to increased rates relative to other states. Ensuring that individuals are educated about this topic is an international public health initiative that should not be taken lightly. • • It’s been really nice moving home, but I’ve really been missing my Hawai’i life lately. I can’t believe it’s come to an end so quickly. I miss the warm weather, being close to the beach, community outreach events like this and the DKICP Health Fair, but most of all my classmates! We were in Zoom workshop last week w/ mandatory video-on, it was so nice actually seeing all of my them even if it was via Zoom. • It was so easy to take foregranted all these little things in my life before, and now it’s already almost the end of my P3 year! Take advantage of the time you spend in pharmacy school, virtually study w/ your classmates * make the best of this situation! • We’ll get through this together! ✨

In the midst of this global pandemic, I wanted to take the time to share my thoughts. This has been a very rough time for many people, including those who have fallen ill to Covid-19 and all the healthcare workers tirelessly aiding them. Pharmacies have become bombarded, and it’s been an emotional rollercoaster to say the least. Society has also taken a huge hit with having to adjust to quarantined life. It’s been rough for all of us. One thing that has helped me through all this has been realizing how crucial my role is as a pharmacy intern. Yes it may be a small role, but I appreciate that role more than I ever have before. Compassion and patience is all we can practice in our lives right now, hoping that this all ends soon. Throwback Thursday to my P2 Summer Hospital Rotation @ Pali Momi Hospital in Aiea, HI (August 2019). • This was a 2 week rotation, M-F, 0600-1500 (fun fact: my preceptor=military!). Our day would begin with mini rounds, followed by topic discussion, and then grand rounds w/ the collaborative healthcare team. We’d usually have a working lunch, where we would be working on our SOAP presentation on a patient we were assigned on Day 1 of rotations during which our preceptor was generally in a meeting. • Mini rounds included myself, one of my P2 classmates, and an M4 from JABSOM. Grand rounds included Attending (changed weekly), 1 pharmacist (my preceptor), 3 medical residents (PGY-1, PGY-2, PGY-3), 1 pharmacy resident (PGY-1), 2 fourth year medical students (M4’s), and 2-3 second year pharmacy students (P2’s). I really enjoyed working on this collaborative healthcare team and am really looking forward to my Acute APPE Rotation! • There are 4-5 different major hospital sites on the island of Oahu that host pharmacy students from my school (DKICP) that come together every Wednesday afternoon at the Gold Bond Building at JABSOM. This usually consists of P4’s and P2’s and this is where I gave my final SOAP presentation in order to pass the rotation. • Honestly, I’m just hoping that everything is going to go as currently scheduled. • 👩🏻‍⚕️
